Your Cells Don't Care If You're Happy. They Care Why 🧬

Two people... Same stress level. Same income. Same social life. Both report being roughly happy with their lives.
One of them is producing a gene expression profile that lowers inflammation, strengthens immune defence, and is associated with meaningfully reduced risk of cardiovascular disease, dementia, and all-cause mortality.
The other is producing the same molecular/genetic signature as someone under chronic threat.
Same subjective experience. Opposite biology.
The difference? Where their happiness is coming from.
I know what you're thinking but this is not a philosophical observation. It is a finding from the genetics laboratory at UCLA, replicated across multiple studies, and it reframes purpose from a motivational poster concept into something that belongs in the same conversation as sleep quality, HRV, and cortisol management.
We covered the dopamine edition and the cortisol edition as critical performance neurochemicals. Purpose belongs in that conversation. Not as a soft add-on, but as a hard biological input.

The Basics
Psychology has long drawn a distinction between two types of well-being. Hedonic well-being is the one most people are optimising for: pleasure, comfort, life satisfaction. How much are you enjoying yourself? Eudaimonic well-being is different. It comes from purpose, meaning, contribution, and personal growth. Are you living in a way that expresses who you actually are and matters beyond your own comfort?
Aristotle distinguished between these two roughly 2,400 years ago, arguing that the "good life" was not about maximising pleasure but about living in alignment with your highest potential. Modern psychology formalised this through Carol Ryff's work in 1989, and the empirical case has been building ever since.
Purpose in life, the clearest eudaimonic construct we have, is defined by researchers as "a self-organising life aim that stimulates goals, promotes healthy behaviours, and gives meaning to life" (Alimujiang et al., 2019). Not a vision board or a mantra. A coherent internal compass that shapes how you allocate attention, energy, and effort.
What is remarkable is how much that compass matters biologically.
The Numbers
Let's build the case properly before we get to mechanisms, because the mortality data alone warrants your attention.
Cohen, Bavishi & Rozanski (2016) conducted a meta-analysis of 10 prospective studies covering 136,265 people. High sense of purpose was associated with a 17% reduction in all-cause mortality and a 17% reduction in cardiovascular events (after adjusting for known cardiovascular risk factors). That is not a marginal finding. For context, that effect size competes favourably with several pharmaceutical interventions for cardiovascular disease.
Alimujiang et al. (2019), published in JAMA Network Open, followed 6,985 adults from the Health and Retirement Study. Those with the lowest life purpose scores had more than double the mortality of those with the highest scores (a fully adjusted hazard ratio of 2.43) over the follow-up period.
Hill & Turiano (2014) used MIDUS data (N = 6,163) and found purpose predicted reduced mortality risk across all age groups. You get the idea. A sense of purpose seems to protect against major causes of death across all age groups.
On the cognitive side, Boyle et al. (2012) at Rush University made one of the most striking observations in the entire ageing literature: purpose modified the relationship between Alzheimer's pathology and cognitive function. People with high purpose maintained better cognitive performance even when their brains showed the same level of plaques and tangles as people with low purpose who had dementia. Their brains were equally damaged. Their function was not. A 2025 UC Davis study (N = 13,000+) confirmed a 28% lower risk of cognitive impairment for those with a strong sense of purpose, a protective effect that held even among those carrying the APOE4 Alzheimer's risk gene.
Before I get to the genomics finding one note worth making: large surveys suggest that only a minority of adults feel they have a clear, well-defined sense of purpose. Meaning many of your colleagues, your team, and probably some version of your past or present self are operating under what the research identifies as a significant health and performance liability.
The Where
Being a neuroscientist by training, I am deeply interested in how 'soft' concepts like this manifest in the brain. If you're not interested in the neuroscience behind it, skip this section.
The prefrontal cortex (PFC), specifically the medial and ventromedial PFC, is where purpose gets operationalised. This is your goal-setting, value-based decision-making, and future-simulation territory. When you have a coherent purpose, the PFC has something to orient around, a persistent, high-level schema that organises everything below it (this is the "self-organising life aim" from above). When purpose is absent, the PFC carries a much higher cognitive load, running the "what should I be doing with my life" background process in parallel with everything else you're asking it to handle. That tax is real, and it shows up as decision fatigue, difficulty with strategic thinking, and a chronic low-level sense of drift.
If you've been reading my newsletter a while you'll know this region. The Default Mode Network (DMN) (the medial PFC, posterior cingulate cortex, and angular gyrus) is the network that activates during self-referential processing, autobiographical memory, and future planning. We covered the DMN recently in the boredom and cognition research, but the purpose connection is this: people with higher meaning in life show greater connectivity within DMN nodes and better cross-network integration. Their brains are better at connecting self-knowledge with future direction. The internal GPS has a destination.
The ventral striatum is your reward valuation centre. What is interesting here is that purpose-driven activity engages this region differently from pleasure-driven activity. The signal is more sustained, less spiky, and more resistant to habituation. This is the neuroscience of why meaningful work doesn't bore you the same way comfort-seeking does. It connects directly to the dopamine discussion: purpose tends to generate tonic (sustained, moderate) dopamine activity rather than phasic spikes that crash. You are running on a cleaner, more consistent fuel source.
The How and Why (the Genomics Finding)
Here is where the research on purpose gets really interesting.
Steve Cole's laboratory at UCLA had previously identified something called the Conserved Transcriptional Response to Adversity (CTRA). A characteristic gene expression pattern that appears under chronic threat, social isolation, and prolonged stress. The CTRA profile looks like this:
- Upregulation of pro-inflammatory genes (particularly the NF-κB pathway)
- Downregulation of antiviral response genes (Type I interferon)
- Downregulation of antibody synthesis genes
In plain language: your immune system shifts into a wound-healing, inflammatory posture, preparing for physical injury, at the expense of viral defence and antibody production. This is the molecular signature of chronic threat. It is associated with accelerated cardiovascular disease, impaired immune function, and increased cancer risk.
In 2013, Fredrickson and Cole published a study in PNAS that applied the CTRA framework to well-being. They split participants into those high on eudaimonic well-being (purpose, meaning, growth) and those high on hedonic well-being (pleasure, comfort, life satisfaction), and then examined their gene expression.
The eudaimonic group showed decreased CTRA expression. Lower inflammation, stronger antiviral and antibody responses. Biological markers of resilience.
The hedonic group showed increased CTRA expression. The same inflammatory molecular profile seen in chronic adversity.
Here is the interesting part that changes the conversation: both groups reported feeling equally happy. The subjective experience was identical. The molecular reality was not.
The protective eudaimonic side of this finding was replicated in a larger 2015 study. The key variable appeared to be not how happy people felt, but the ratio of purpose-driven to pleasure-driven happiness.
A note on these findings: this finding has faced methodological critique (Brown et al., 2014) and the specifics remain debated. But the CTRA framework itself is very well established, and the directional finding (that eudaimonic and hedonic well-being have different biological signatures) has held up across subsequent work. Treat it as robust, not settled. That nuance does not diminish the significance of the question it is asking us.
The Problem: How Senior Leadership Erodes Purpose
The executives I work with are not, on the whole, people who have never had a sense of purpose. They are people whose purpose has been gradually, systematically displaced, often while they were busy being successful.
A few mechanisms to think about:
Target fixation. When the primary measure of a good quarter is a revenue number or a board-approved KPI, meaning gets replaced by metric. The "why" becomes "because the board expects it." This is a clean example of hedonic-to-eudaimonic substitution, external reward replacing intrinsic meaning. The CTRA data suggests this substitution has a cost your bloodwork might already be recording.
Calendar saturation. Purpose requires reflection. Genuine DMN engagement, sitting with the question of what matters and why. Calendars packed to 90% capacity eliminate the reflective space that allows purpose to stay connected to daily life.
Identity fusion with role. When "who I am" becomes indistinguishable from "what I do," purpose becomes structurally fragile. A title change, an exit, a restructure... and the foundation disappears. The leaders who transition well are those who built their sense of meaning on something portable, something in them. Many don't discover this until they are forced to.
The hedonic treadmill. High achievement creates a dopamine cycle built around reward events: the deal, the promotion, the exit. These are by definition hedonic (pleasure and satisfaction from attainment). That is not bad in itself. But when the majority of a leader's positive emotional life is sourced from achievement-based pleasure rather than purpose-based meaning, the CTRA data suggests that this will be taking a hidden (for now) toll.
Three Ways to Start Moving the Needle
These are not life overhauls. They are deliberate, small shifts that will give you data about where you are and start moving the compass needle. Think of them as diagnostics that double as interventions.
1. The Peak Experience Scan
The Science
The values and purpose research consistently shows that who you actually are (your genuine purpose, your real values) is already visible in your lived experience. It shows up in the moments when you felt most alive, most yourself, most in the right place. These are not always the big career wins. Sometimes they are a conversation, a problem you solved, a time you helped someone. The pattern across those moments points toward what Carol Ryff and her colleagues have identified as the core eudaimonic signals: autonomy, growth, contribution, authentic expression.
Identifying these patterns is the first step in the Life Crafting methodology (Schippers & Ziegler, 2019), which is the most fully specified structured purpose-discovery protocol I am aware of. This is the starter version.
The Protocol
Set a 25-minute timer. No devices. Pen and paper (research by Mueller & Oppenheimer suggests handwriting may prompt deeper processing than typing, though direct replications have been mixed). Write down three moments from your life (any age, any context) when you felt genuinely alive and fully yourself. Not "successful." Not "impressive." Alive, authentically you, and engrossed in the moment. For each, answer: What were you doing? Who was there? What value was being expressed? What made it meaningful rather than merely enjoyable?
When all three are written, read them back and underline any recurring words or themes. Do not force a conclusion. Just notice what appears more than once.
Expected Outcome
Most people find this confronting in a useful way. Either the recurring themes are present in their current life and they had forgotten that, or they are conspicuously absent and they now have something concrete to act on. Either way, you leave with data. Do this once. It takes 25 minutes. It produces disproportionate clarity relative to the time investment.
2. The Eudaimonic Check
The Science
The Fredrickson and Cole CTRA finding hinges on the ratio of eudaimonic to hedonic well-being, not just the presence of one or the other. Hedonic activity is not the enemy here (rest, pleasure, and comfort are biologically necessary). The problem is hedonic predominance: when the majority of your positive emotional life is sourced from consumption and comfort rather than from meaning and contribution.
The Eudaimonic Check is a tracking tool for that ratio. Harkin et al. (2016), in a meta-analysis of 138 studies covering nearly 20,000 participants, found that self-monitoring of goal progress significantly improves outcomes (d = 0.40). The act of noticing changes behaviour, often without any additional intervention (see the RAS newsletter edition for more on this).
The Protocol
Each evening this week answer one question in writing. It takes two minutes:
"Was today's sense of satisfaction primarily from meaning (something I made, contributed, grew through, or expressed authentically) or from comfort (something I consumed, escaped into, or found easy)?"
You do not need to score it or create a rubric. Just write an honest sentence or two and notice the pattern across the seven days. At the end of the week, look back across the seven answers. What do you see?
If your week was predominantly comfort-sourced, that's fine! This is useful data about what your operating system is currently running on, and what the CTRA research predicts your biology looks like as a result. That data is worth having.
3. The Obligation Audit
The Science
The research on self-concordance (Sheldon & Elliot, 1999) demonstrates that the degree to which your activities align with your authentic values predicts not just well-being but sustained effort and cognitive engagement. What is perhaps more relevant for this audience is the flip side: activities that conflict with your values activate what is called the anterior cingulate cortex, your brain's conflict-monitoring system, producing a chronic low-grade stress signal even when you are not consciously aware of the mismatch.
In practical terms, you may have obligations in your current week that are draining not because they are difficult, but because they are misaligned. Every such obligation is a background load on your stress physiology. Naming them is the first step to making a conscious decision about each one.
The Protocol
Look at your calendar and task list for the coming week. For each major commitment (meeting, project, obligation) ask a single question: "Does this connect to anything I care deeply about?". Not "is this productive" or "is this important to someone else." Does it connect to anything you care about?
Flag the items where your honest answer is no. Do not act on any of them yet. Simply flag them and count how many there are. Then, for each flagged item, ask one follow-up question: "What would actually happen if I said no to this, delegated this, or significantly reduced my time on it?"
Write down the actual consequences, not just the feared, worst case consequences, but the real ones. For most flagged items, the honest answer is: very little would happen. Someone else would do it, it would not happen and no one would notice, or the consequences are manageable. That realisation is the beginning of reclaiming agency over your calendar. This one may be tough and it is not about suddenly saying no to everything but being aware of what you're saying yes to (which then extends to what you're saying no to).
The Bigger Picture
The three practices above will take you a combined total of roughly 45 minutes this week. They will not transform your life. That is not what they are designed to do.
What they will do is give you three things the research consistently identifies as prerequisites for purpose development: awareness of your values, visibility of the gap between stated and lived purpose, and the beginning of a data set about where your daily satisfaction is actually coming from.
For most people, that data alone starts something. Because purpose does not emerge from a grand revelation, it usually emerges from accumulated small observations about what energises you, what drains you, and what you keep returning to when no one is watching. The important thing is start noting these down to increase your awareness so you can become more intentional about this.

- None of these require you to overhaul your life. They just require you to look clearly at it.
